You worked hard for it. You changed your eating, stayed consistent for months. The number on the scale finally moved — significantly. You should feel triumphant.

But two to four months after the weight started coming off, something unexpected happens: your hair starts coming out in amounts that frighten you. The brush. The shower. The pillow.

This is one of the most underreported side effects of significant weight loss — and it doesn't just affect crash dieters. It affects women on medically supervised programs, women recovering from bariatric surgery, and increasingly, women on GLP-1 medications (semaglutide, tirzepatide) who are losing weight effectively and finding hair loss to be the thing nobody warned them about.

What's actually happening? The shedding is generally attributed to the rapid weight loss itself and the reduced caloric and protein intake — a recognized telogen effluvium trigger — rather than a direct effect of the medication on the follicle; whether any direct effect exists is an open research question. Practically, that framing matters: GLP-1 medications suppress appetite across all foods, including protein. A patient eating 900–1,100 kcal/day can easily land far below the protein intake follicle keratin synthesis requires — while also running down iron, zinc, and vitamin D.

Women recovering from bariatric surgery face the same mechanism at maximum intensity: dramatic caloric restriction, plus — depending on the procedure — genuine malabsorption of protein, iron, zinc, and B12. Clinical guidelines for bariatric care specify structured, lifelong nutritional supplementation and monitoring for exactly this reason (Mechanick et al., 2013).
